The main reception desk is open during lunchtime to the public should you wish to collect an application form or leave a message.
Please remember that the closing date for receipt of application forms is the 31st of August 2010.
Failure to submit your fully completed application by the closing date will delay processing and payment of your maintenance grant.
You are advised also that no maintenance grant payments are made before October 2010.
County Clare VEC third level grants office would like to assure all grant applicants of the strictest confidence in all matters pertaining to their grant applications.
Documentation Required for Assessment Purposes:
Self-Employed/Farmers:
Set of Accounts for full tax year 2009 / Computation of Profits
Notice of Assessment for 2009
Or
Letter of Exemption if exempt from tax and your own set of accounts.
Paye Income:
P60 for 2009 (every employee should have a P60 from their employer for 2009)
If you are unable to submit your P60 you must submit a letter from your employer for gross income earned by you in 2009.
Please note you must submit documentary evidence for all income earned in 2009.
You must also submit your P21 Balancing Statement for 2009, (this document is available from the tax office on request).
If you ceased employment with your employer in 2009 you must submit your P45.
Please note you must back up all income listed on P21.
Social Welfare Income:
If you were in receipt of a social welfare payment in 2009 you must apply to the relevant social welfare section for a letter stating the type of payment you were in receipt of, the length of time on the payment and the total amount you received in benefit for the year ended in December 2009.
Separation Declaration:
For those who are separated/divorced or single we would ask that they supply a maintenance agreement/order/divorce decree or an affidavit sworn before a Commissioner for Oaths that he/she is separated.
Birth Certificate/Baptismal Certificate:
We request that applicants furnish us with their original long birth certificate or baptismal certificate. Any originals that we receive will be photocopied and returned as soon as possible.
Passports:
Final Course Acceptance Form:
With each application that we receive we require a Final Course Acceptance form which the candidate should fill out once they know they have been accepted on the course. This form is also supplied in the application pack. Please note that we would ask candidates to apply whether they know if they have been accepted on their chosen course or not. The Final Course Acceptance form can be submitted at a later date.
Please note Form FA2 is for PLC Applicants ONLY Form FA1 is for all other applicants completing levels 7, 8 and 9 courses. These forms do not need to be stamped by the School/College that the student is attending.
Checklist:
We would also ask candidates to refer to the checklist on the back of the application form. Please ensure that you have supplied everything that applies to you. It is imperative that the application form is signed by the candidate and their parents (if applicable). If the application form is not signed we will have to return the form for signature before we can assess it.
PPS Numbers:
In order for an application to be assessed we need the applicants PPS number. We cannot proceed with the application until this is supplied.
Not/Applicable:
If something on the form does not apply to you please do not just leave it blank. We would ask that candidates put a line through the box and write N/A over it.
Permanent Address:
When submitting an address for correspondence, please ensure that it is your permanent address. This is to ensure that any correspondence that we send to you will be received.
Family Members:
Please state if there are other family members attending third level education and living at the same address as you.
Previous Qualification:
Please disclose any previous third level courses attended and the qualification (if applicable) received. Back up documentation will be required as proof.
Summary of Reckonable Income:
On page 8 of the application form there is a table where the summary of reckonable income is to be filled. It is very important that you fill in this in to the best of your ability.
